Approved caterer waiver form

Caterers and food truck vendors must meet specific requirements in order to serve food on University property. If you require a caterer that is not on the pre-approved caterers list, you must submit this form to the University Sanitarian’s Office at least 30 days prior to your event. Submitting the application is not a guarantee that your caterer will be approved.

Instructions

This form must be filled out and sent to the Sanitarian’s Office at least 30 calendar days prior to any catered event that is not using a pre-approved caterer. The application is not required for “drop off” food for less than 50 people who are all University staff but all University requirements for food vendors must still be met.

A health permit and insurance may be required. Waivers are not granted unless it is clear that approved caterers are not able to meet your group’s needs. All decisions will be provided in writing.

The approval processing fee is $30.00. There is an additional $10 fee for late applications.

Submitting this form early in the process can help your organization have a successful event by utilizing University resources that can assist you. Failure to pay

Failure to pay for the permit and/or failure to acquire the permit itself, will disqualify that organization from being granted another temporary food event until the permit is paid for and received by that group or organization.

To complete this request you will need to email the following information to the sanitarian:

Insurance Information

• Certificate of Insurance, naming the University of Rochester as an additional insured, evidencing the following insurance:

o General Liability Insurance with $1 M per occurrence/$2M aggregate
o Auto Insurance with $1M in coverage
o Worker Compensation Insurance as required by NYS
o Disability Insurance as required by NYS

  • Copy of Monroe County issued catering permit
  • Copy of current Food Safety Certification for at least one food handler
