Reporting injury, illness, and near miss events
When an injury or near-miss event or illness occurs on University property, it should be reported through the appropriate incident reporting system. Descriptions of systems and instructions are included for reporting and investigating incidents. OSHA reporting requirements for severe employee injuries are also included.

Requirements
When an injury, near-miss event (an unplanned event that did not result in injury, illness, or damage – but had the potential to do so), or illness occurs, or you witness or are involved in a near-miss incident, it should be documented through one of the reporting systems above within 24 hours of the occurrence.
Upon submission of the report, Leave Administration, Environmental Health & Safety, and the employee’s immediate supervisor will be notified.
All blood exposure needlestick incidents (or injuries) should be reported via the Blood Exposure Hotline at 275-1164 as well as through the Medical Center on-line reporting system.
Information for employees
Employees (or a supervisor) should fill out an incident report within 24 hours to comply with all state and federal regulations. Environmental Health and Safety will respond with corrective actions once a supervisor review has been filed.
- Login to the Employee Reporting System using your UR username and password.
- Click on the “Report Event” option in the top right corner.
- Choose which of the following forms you’d like to fill out:
- Employee Injury/Illness
- Near Miss/Hazard
- Ergonomic Concern/Request
- University Motor Vehicle Accident
- Fill out the form to the best of your ability. All yellow fields and fields denoted by a red asterisk (*) are required.
- Select the appropriate information for fields that have a tree icon by clicking on the icon and then expand the section by clicking on the (+) icon, or collapsing the section by clicking on the (-) icon.
- For fields that have the magnifying glass, click on the magnifying glass and select the code that matches the appropriate information. Do not click in the box as that will give you a limited number of choices. Clicking on the magnifying glass will give you all the options for that specific field. Fields that do not have a or are a free form field that allows you to answer the question without any character limitations.
- Once you are done, press the “Submit” button at the upper left corner of the page.
A near miss is an unintentional incident that could have caused damage, injury or death but was narrowly avoided.
In the context of safety, a near miss may be attributed to human error, or might be a result of faulty safety systems or processes in an organization.
Incidents must be reported within 24 hours to comply with all state and federal regulations.

You must report all incidents, regardless of severity. If there was no injury, use the Near Miss/Hazard form.

employee/contractor, visiting researcher, or UR student, and do not have a Net ID,

Please see your supervisor for a printed copy of your incident report.
Your supervisor of record is now automatically notified of the incident report being submitted. If someone else in the department needs a copy, direct them to your supervisor.

No. Once an incident has been reported and submitted, you’re all set. Your supervisor
will be notified of the incident, as well as Worker’s Compensation, and Environmental
Health and Safety.
Information for supervisors conducting reviews or investigations
Supervisors should conduct reviews within 48 hours of an incident whenever possible. Environmental Health and Safety can assist with the investigation and will respond with corrective actions once your report has been filed.
Employees and supervisors are the first line of defense to correct any potential issues or enact any procedures that will help ensure a safe environment.

- Employee completes and submits incident form.
- The supervisor reviews the incident and initiates a review/investigation to help reduce the chance of similar incidents from happening to someone else.
- Environmental Health and Safety reviews and assists the supervisor with the
investigation. - Corrective actions or changes are implemented if needed.

No. Each page must be saved by clicking save in the top left corner.
Yes. Supervisors are allowed to add their own comments and surrounding details to the incident report after it is filed. The original incident report submitted by the employee, will be retained as a permanent record and unable to be changed.
Note: You CANNOT cut and paste external documents into the application. You must type directly into the application and use the look up tables and the trees to select your field choices.
Yes. When you click the action button at the top next to the save button, there is a “Print Form” button that will generate a print friendly copy of the whole report.
Yes. Supervisors can see all events that are assigned to them. Employees will only be able to view the report that they filed.
It is possible to have the Supervisor or department designated safety person fill out the
incident form. Simply fill it out using the employee’s information and the report will go
to the injured employee’s direct Supervisor, not the individual submitting it.
Incident reports are maintained electronically in the system. The supervisor of record automatically receives notification of all incidents reported and can print a copy of the report for someone else in the department upon request.
Employee severe injury reporting
All injuries resulting in inpatient hospitalization or death are required by OSHA to be reported within 24 hours or 8 hours respectively.
Initial notification shall be done in person or by telephone by calling Public Safety at 275-3333.
Information regarding severe employee injuries
The Occupational Safety and Health Administration (OSHA) requires employers to report all work-related fatalities within 8 hours and all in-patient hospitalizations, amputations, and loss of an eye within 24 hours of finding out about the incident. These types of injuries are categorized as “severe“.
According to OSHA the updated reporting requirements are not simply paperwork but have a life-saving purpose: they will enable employers and workers to prevent future injuries by identifying and eliminating the most serious workplace hazards.
University managers and supervisors must notify Public Safety immediately after discovery and ensuring the injured party is cared for when an employee experiences an injury defined by OSHA as severe. Initial notification shall be done in person or by telephone by calling Public Safety at 275-3333. It is not acceptable to leave a voice mail, page or email.
The person notifying Public Safety must provide information about the injury, including but not limited to:
- Location of the work-related incident (e.g. specific campus, building, room, etc.)
- Time of the work-related incident
- Type of reportable event (i.e., fatality, in-patient hospitalization, amputation (part of body) or loss of an eye)
- Number of employees who suffered the event
- Names of the employee who suffered the event
- Contact person and his or her phone number
- Brief description of the work-related incident
Upon learning of the incident, Public Safety will immediately report the incident to EHS, providing the information received. EHS will make the necessary notification to OSHA.
